  • Employment Type: TravelJOB TITLE: Registered NurseJOB CODE: 5203DEPARTMENT: HospicePAY GRADE:IMMEDIATE SUPERVISOR: Hospice ManagerDEFINITION:Provides quality patient care in the home setting utilizing the nursing process and accomplishes indirect patient/office related work in accordance with the policies and procedures of the Home Health Department.EDUCATION:I sssREGULATORY REQUIREMENTS (IF APPLICABLE):Authorized to practice as a Registered Nurse in the State of Oklahoma.MINIMUM QUALIFICATIONS:7& I PREFERRED QUALIFICATIONS:Previous experience in mental health facility, with basic knowledge.Knowledge of patient management and techniques for physical intervention.Satisfactorily completed the hospital orientation program, meet additional credentials for special care unit as indicated in skills checklist.PRINCIPLE RESPONSIBILITIES:Participates and implements age specific care and interaction.Employee has attended departmental orientation (training) and annual inservice education for age specific training and implements in work area what was learned in age specific training.Makes the initial evaluation visit to determine whether care is needed; type, duration, and frequency of care; and, to determine whether needed care can be safely and appropriately provided by the Hospice.The initial evaluation is completed within 24 hours of patient admission to services. Literature will be left in the patient's home for reference as needed.Provides patients, family, and/or caregiver with accurate information regarding scope of Home Health Services.Patient has reference material related to Hospice.Re-evaluates the patient's nursing needs on a continual basisTreatment plans are individualized to reflect current patient needs/problems and using nursing diagnosis format.Involves the patient and/ or significant others to the fullest extent of their ability and willingness to participate.Patient/family strengths, motivation, and compliance are documented and evaluated at regular intervals.Completes patient visits, as is appropriate to the patient's care needs with "On Call" coverage 7 days per week, 24 hours per day.Documentation of home visits is complete and in the office by end of business day in most instances.Completes daily patient visit /mileage log, list of supplies used and turns in to the Scheduler the next day.Activity sheets are turned in within 1 day of visits so billing can be completed on a daily basis.Informs the physician and other personnel of changes in the patient's condition and needs.Prompt physician contact is documented. Telephone orders are implemented without error and signed.Prepares care plan for Home Health Aide and supervises the care provided by the Hospice Aide effectively and in a timely manner.R.N supervisory aide visits of Hospice Aides are conducted every 2 weeks and documented in the patient's record.Recommendations and requirements for additional Hospice Services for clients are communicated to appropriate individuals in a timely manner.Appropriate documentation is clipped together with orders for any auxiliary services within 24 hours.Demonstrates basic knowledge of law and legal aspects applicable to nursing practice.All necessary consents for treatment or completion of visits are properly executed.Communicate with Manager and staff members of patient activity and documents pertinent information clearly and accurately.Patient needs are met within the written objectives of the patient treatment plan.Participates in patient coordination of care meetings.Changes in patient clinical status are updated. Discussion with discharge planning as an ongoing part of patient care.Participates and assists with orientation of Hospice staff members.Orientation of staff members and teaching students as requested.15.
