Job Description & Requirements - Specialty: Acute Care Case Management - Discipline: RN - Duration: 12 weeks - 32 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Description: Travel – RN Lead Case Manager Facility: Mercy Rehab OKC South Assignment Type: Travel Duration: 12 weeks Schedule: Days Start Time: 8:00 AM End Time: 4:30 PM Years of Experience: Minimum 2 years of social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred. Previous leadership or supervisory experience strongly preferred. Position Summary The Lead Case Manager oversees case management operations within the rehabilitation setting, ensuring effective discharge planning, interdisciplinary coordination, and compliance with regulatory standards. This role provides leadership support, manages workflow, and serves as a resource for complex clinical and psychosocial cases. Key Responsibilities - Lead and support the case management team in daily operations - Coordinate patient assessments, discharge planning, and transitions of care - Collaborate with physicians, nursing, therapy, and ancillary departments - Ensure compliance with CMS, state, and facility regulations - Review patient progress, barriers to discharge, and resource needs - Provide guidance on complex psychosocial, financial, and placement issues - Maintain accurate documentation and timely communication - Participate in performance improvement initiatives and staff development - Serve as a liaison between patients, families, and the care team Required Certifications and Education - Current Registered Nurse (RN) license or Social Work licensure - OR Master’s or Bachelor’s degree in Social Work - Certification in Case Management or Rehabilitation preferred, including: - Commission for Case Manager Certification (CCM) - Association of Rehabilitation Nurses (ARN) certification - American Case Management Association (ACM) - ANCC Board Certification in Case Management (e.g., RN?BC) Experience Requirements - Minimum 2 years of social work or case management experience in an inpatient setting highly preferred - Acute care or rehabilitation hospital experience preferred - Previous leadership or supervisory experience strongly preferred
