EMR Documentation System: Kinnser/WellSky Pay: Competitive, based on experience Our Admissions Case Manager is a member of the Intake department who follows admissions through completion with full and complete documentation. They communicate with co-workers, field staff, patients and coordinate follow up needed to obtain the necessary documentation for a complete patient record. Carter Offers: Portions of Your Health Insurance Premiums PAID BY Carter Flexible Hours Generous Paid Time Off Package (Including 6 Holidays) Paid Birthdays Off Responsibilities: Receive referrals, via telephone, FAX or in person; processes them, secures physician orders, coordinates all necessary disciplines and maintains an accurate record of all referrals. Collects at time of referral. Collects all data required by policy and procedures at time of referral, verifies insurance information and verifies home care coverage. Reviews clinical documentation to determine reason for home health to communicate with field staff and coordinate documentation. Receives authorization from home Health Services. Evaluates referral against customer needs, determines appropriate services for the patient. Establishes individual patient’s multi-disciplinary needs. Verifies funding sources for new patient referrals and coordinates payment issues with appropriate affected persons. Qualifications: Two years in case management or intake, highly desirable. Knowledge of Medicare, Medicaid, private insurance and DME regulations, as well as JCAHO, DHS, local, state and federal regulations Broad knowledge of principals of nursing and rehab home health and their application. Excellent Verbal and written communications skills including excellent telephone communication and listening skills. Ability to seek and obtain patient information. Ability to multi-task and manage multiple patient cases.
