Job Description Social Worker (Hybrid) Location: Chattanooga, Tennessee On site: Hybrid – 3 days onsite / 2 days remote Duration: 3-Month Contract with high potential for extension and/or conversion to a full-time position Calculated Hire is seeking a Social Worker to support a Clinical Care Team by helping patients navigate social services, healthcare resources, and community support programs. This role works closely with primary care providers and nurses to address social determinants of health (SDOH) and ensure patients have access to the resources they need for successful care. Key Responsibilities Conduct social needs screenings and assessments for patients referred by primary care providers Assist patients with accessing community resources related to housing, food, mental health services, and other social supports Coordinate care with providers, nurses, hospitals, and community organizations Support discharge planning and follow-up care for patients Help patients navigate specialty referrals, imaging appointments, and medical records requests Assist patients in identifying low-cost prescription options and applying for patient assistance programs Maintain and update resource lists for community services and specialty referrals Document patient interactions and care coordination activities in patient records Additional Responsibilities Participate in clinical team meetings and provider meetings as requested Build relationships with local social service agencies and community partners Identify barriers to care and help develop solutions to better support patients Qualifications 2 years of experience in Social Work or a related field Bachelor’s Degree preferred Associate’s Degree with relevant experience will be considered Strong communication, care coordination, and community resource navigation skills
